The State is issuing a Request for Proposal (“RFP”) seeking a qualified Vendor to provide the following dental programs and/or services: a self-funded Dental Preferred Provider Organization Plan (“PPO”) and/or a fully insured Dental Health Maintenance Organization (“DHMO”), and/or a Dental Discount Program.
