Reference Special Notice W81K04-16-R-0003 dated 18 Dec 2015. The Health Readiness Contracting Office (Provisional) (HRCO – (P)) – Joint Base San Antonio, Fort Sam Houston, Texas intends to award a sole source, firm-fixed price (FFP) contract to Millennium Dental Technologies (MDT) Inc., located in Cerritos CA, DUNS: 062754457, Cage Code: 4FZA9. Millennium Dental Technologies Inc. is the
developer and manufacturer of the PerioLase MVP-7 Laser and patent holder of the LANAP protocol. The period of performance will be 5 (five) years from date of contract award. The statutory authority permitting acquisition of this
requirement by other than full and open competition is 41 USC 254(c)(1) as implemented by FAR 6.302-1 (Only one responsible source and no other supplies or services will satisfy the agency requirements).
Millennium Dental Technologies Inc. is the only responsible source capable of satisfying the Dental Directorate’s requirement for dental equipment and training necessary to perform the Laser-Assisted New Attachment (LANAP) protocol to treat moderate to advanced periodontal disease and/or peri-implantitis. The North American Industrial Code System (NAICS) code is 334510 with a size standard of 500 employees.
