BRAND NAME REQUIREMENT
1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
2. The solicitation number is FA3047-15-T-0014 and is issued as a Request for Quotation (RFQ).
3. The solicitation document and incorporated provisions are those in effect though Federal Acquisition Circular 2005-83 and DFARS change notice 20150626.
4. This is a notice that this order has no small business classifications and is for Full and Open Competition, with brand name specifications. The NAICS code is 339114 and the size standard is 500 employees.
5. The contract line items numbers and items, quantities and units of measure pertaining to this requirement are identified on the solicitation. The solicitation with statement of work is available for download via www.fbo.gov. Quotes shall be on an all or none basis.
6. The contractor shall supply all personnel, equipment, tools, materials, supervision, and all other items or services necessary for delivery and installation of all equipment and for training as in the Performance Work Statement.
7. Delivery location is Dunn Dental Clinic, 1615 Truemper St., JBSA-Lackland and all items shall be FOB destination. Delivery date shall be 30 days after receipt of purchase order.
8. Submission of quotes must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Items. Written quotes are required. This is a brand name solicitation, all quotes must meet the exact brand name specifications, and basis for award will be low price.
9. Quoters must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with their quotes.
10. To be eligible to receive an award resulting from this solicitation, registration with the System for Award Management (SAM) must be current, no exceptions. The System for Award Management is the DOD Central Contractor Registration (CCR) database. To register, please visit https://www.sam.gov/portal/public/SAM/.
11. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition to include Addendum 52.212-4 details of which are included in the solicitation.
12. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes Or Executive Orders - Commercial Items, applies to this acquisition details of which are included in the solicitation.
13. The vendor shall submit the response to this RFQ via email to the Contract Administrator: Kirsten Bucheli, (210) 671-5905, E-mail: kirsten.bucheli@us.af.mil; no later than 12:00 noon CST on August 26, 2015 in order to be considered timely. Any quote, modification, revision, or withdrawal of a quote received after the exact time specified for receipt of quote is "late" and will not be considered.
