The Carrollton-Farmers Branch Independent School District offers four self-funded medical plans and one self-funded Dental plan to its employees and dependants. The self-funded plans are administered by United Healthcare. As of June 2007, there were 2,513 enrolled in the four self-funded Medical plans and 1,216 enrolled in the self-funded Dental plan. The four Medical plans are PPO plans, which vary by deductible and coinsurance. The District accesses the United Healthcare Network. The District currently provides Basic Life Insurance and AD&D to employees enrolled in the Medical plan in the amount of $20,000 and $40,000 for employees who do not enroll in a medical plan and is paid for by the District. The current Basic Life and Voluntary Life Insurance & AD&D are administered by MetLife. The current Voluntary Vision Plan is administered by Vision Service Plan with 958 participating employees. The Voluntary Disability Program is being administered by Unum Provident Insurance Company. The District offers a Voluntary Cancer and Specified Disease Program through AIG with 142 participating employees.
